Multiple mucocele yang disertai pyogenic granuloma pada mukosa bibir serta penatalaksanaannya

Abstract
ABSTRAK
Pendahuluan: Pyogenic granuloma (PG) merupakan suatu lesi jinak non-neoplasma vaskuler proliferatif yang biasa terdapat di bibir dan bisa berhubungan dengan faktor hormonal seperti kehamilan. Mucocele merupakan suatu lesi jinak mukosa oral yang berkembang sebagai akibat dari ekstravasasi mukus dari glandula saliva minor yang umumnya terjadi akibat trauma. Laporan kasus: Seorang wanita berusia 30 tahun datang dengan keluhan terdapat benjolan pada bibir bawah. Sekitar 9 bulan yang lalu saat pasien sedang hamil 4,5 bulan benjolan tersebut berbentuk seperti sariawan yang mudah berdarah, kemudian membesar hingga saat pasien melahirkan benjolan tersebut mencapai seukuran stroberi, lalu mengecil hingga saat ini (5 bulan post partus) namun lesi tetap ada. Saat remaja, pasien memiliki kebiasaan menggigit-gigit bibir bawah hingga sering muncul benjolan pada bibir. Hasil pemeriksaan intra oral didapatkan lesi bulat berukuran 1,5x1x1cm, menonjol, hiperemis, berkrusta, dan lunak pada bibir bawah. Diagnosis pasien ini yaitu PG pada labii inferior. Perawatan yang dilakukan pada lesi ini yaitu biopsi eksisi dalam anestesi lokal. Saat intra operasi ditemukan beberapa massa kistik yang dicurigai sebagai mucocele yang juga diambil secara keseluruhan. Spesimen yang didapat kemudian dilakukan pemeriksaan PA dengan hasil Pyogenic granuloma. Saat kontrol 1 minggu, pasien telah mengalami kesembuhan yang baik. PG merupakan lesi yang sering muncul terutama karena faktor hormonal saat kehamilan, seperti pada pasien ini, dimana hormon estrogen perempuan memungkinkan berperan pada ketidakseimbangan antara angiogenesis dan inhibitor. Pasien pada kasus ini memiliki kebiasaan menggigit bibir bawahnya. Simpulan: PG dan mucocele dapat terjadi pada area yang sama, kemungkinan terjadi akibat trauma gigitan juga kehamilan. Penatalaksanaan dengan biopsi eksisi dapat memberikan kesembuhan yang baik, didukung dengan pemeriksaan histopatologi sebagai penentu diagnosis akhir.

ABSTRACT
Introduction: Pyogenic granuloma (PG) is a non-neoplastic proliferative vascular benign lesion that usually grows on the lips, and is thought to be associated with hormonal factors related to pregnancy. Mucocele is a benign lesion of the oral mucosa that develops as a result of extravasation of mucus from the minor salivary glands which usually occurs as a result of trauma. Case report: A 30-year-old woman came with a complaint of a lump on her lower lip since about 9 months ago when the patient was 4.5 months pregnant. Initially the lump was shaped like a canker sore that bleeds easily, then enlarged until when the patient gave birth the lump reached the size of a strawberry, then shrunk until now (5 months postpartum) but the lesion was still there. As a teenager, the patient had a habit of biting his lower lip until he often appeared a lump on the lip. The results of the intra-oral examination revealed a round lesion measuring 1.5x1x1cm, prominent, hyperemic, crusted, and soft on the inferior labii. The diagnosis of this patient was pyogenic granuloma of the inferior labii. The treatment for this lesion was excisional biopsy under local anesthesia. When the lesion was removed, it turned out that at the base of the lesion several cystic masses suspected of being mucocele were found which were also taken as a whole. The specimens obtained were then sent to the Anatomical Pathology section for histopathological examination, with the result is pyogenic granuloma. At 1 week of control, the patient had a good recovery without recurrence of lesions. PG is a lesion that often appears mainly due to hormonal factors during pregnancy, as in this patient, where the female hormone estrogen may play a role in the imbalance between angiogenesis and inhibitors. In this case, the patient had a habit of biting his lower lip. Conclusion: PG and mucocele can occur in the same area, possibly due to bite trauma as well as pregnancy. Treatment with excisional biopsy can provide a good cure, supported by histopathological examination as a determinant of the final diagnosis.
