Faktor resiko tonsiloadenoitis kronis hipertrofi dengan karakteristik ekstra oral wajah pasien

Abstract


ABSTRAK

Pendahuluan: Tonsiloadenoiditis kronis hipertrofi adalah suatu peradangan pada adenoid dan tonsil yang sering terjadi pada anak, serta merupakan penyebab umum terjadinya obstruksi saluran pernafasan atas sehingga mengakibatkan penderita bernafas melalui mulut. Keadaan tersebut dapat menyebabkan terjadinya gangguan dentokraniofasial terutama pada anak-anak karena masih dalam usia tumbuh kembang. Tujuan dari penelitian ini adalah untuk menganalisis faktor risiko tonsiloadenoiditis kronis hipertrofi dengan karakteristik wajah yaitu bentuk kepala, bentuk muka, serta proporsi wajah Metode: Penelitian ini merupakan penelitian analitik cross sectional dengan melibatkan subjek penelitian sebanyak 45 orang, terdiri dari 22 orang pasien terdiagnosis tonsiloadenoiditis kronis hipertrofi dan 23 orang pasien tanpa tonsiloadenoiditis kronis hipertrofi. Data dianalisis dengan menggunakan uji chi-square dan analisis odd ratio. Hasil: Terdapat  faktor resiko pasien tonsiloadenoiditis kronis hipertrofi sebanyak 31,8% memiliki bentuk kepala tidak sesuai nilai rerata (p=0,140 dengan OR 3,111), serta 77,3 % memiliki bentuk wajah yang tidak sesuai nilai rerata normal dan 68,2% mengalami proporsi wajah yang tidak proposional (p=0,099 dengan OR 2,786). Simpulan: Tonsiloadenoiditis kronis hipertrofi dapat menjadi faktor risiko terjadinya perubahan ekstra oral dengan risiko tertinggi potensi perubahan pada bentuk wajah.

ABSTRACT

Introduction: Chronic hypertrophy tonsilloadenoiditis is an inflammation of the adenoids and tonsils that usually occurs children, it also a common cause of upper respiratory tract obstruction, causing patients breathe through the mouth. This situation can cause dentocraniofacial deviation, especially in children because they still in the age of growth. This study aimed was to determine the risk of chronic hypertrophy tonsilloadenoiditis with  the shape of the head, face shape, and the proportion of faces. Methods: This study was a cross sectional analytic study involving 45 research subjects, consisting of 22 patients diagnosed with chronic hypertrophy tonsilloadenoiditis and 23 normal patients. Data were analyzed using chi-square and odd ratio analysis. Result: patients who were diagnosed with chronic hypertrophy tonsilloadenoiditis as much as 31.8% has a head shape with uneven values (p = 0.140 with OR 3.111), 77.3% has facial morphologies with uneven values (p = 0.001 with OR 9.633), 68.2% had a disproportionate proportion of faces (p = 0.099 with OR 2.786). Conclusion: hypertrophy tonsilloadenoiditis can be a risk factor for dentocraniofacial growth disorders with the highest risk of facial  shape changes.
