Penanganan kekurangan ruang untuk pontik gigi premolar pada daerah edentulus yang sempit

Space management for premolar pontic in the narrow edentulous area

Abstract


Pendahuluan: Mengganti gigi yang hilang bertujuan untuk mempertahankan keutuhan sistem stomatognasi sehingga mencegah terjadinya perubahan fungsi akibat adanya pergerakan gigi. Laporan kasus ini mendiskusikan perawatan kehilangan gigi premolar atas kanan dengan ruang pontik yang kurang. Laporan kasus: Pasien pria berusia 20 tahun datang ke Rumah Sakit Gigi dan Mulut Universitas Sumatera Utara (RSGM USU) dengan keluhan sulit mengunyah makanan setelah pencabutan gigi sekitar 6 bulan yang lalu. Pemeriksaan klinis menunjukkan adanya kehilangan gigi 15 dengan daerah edentulus yang sempit. Pemeriksaan radiografi menunjukkan hasil jaringan ligamen periodontal yang normal dan dukungan tulang yang adekuat pada gigi 14 dan 16. Diagnosis yang ditegakkan adalah kehilangan gigi 15 dengan daerah edentulus yang sempit. Rencana perawatan yang dilakukan adalah pembuatan gigi tiruan jembatan dengan bahan porcelain fused to metal. Untuk mendapatkan ruang pontik yang proporsional maka ukuran gigi 14 dan 15 disamakan dengan membagi dua jarak distal 13 ke mesial 16. Berdasarkan pemeriksaan klinis, pemeriksaan penunjang, rencana perawatan, dan diagnosis maka prognosis pasien tersebut adalah baik. Ketebalan preparasi yang terencana, diagnostic wax up, dan komunikasi yang baik dengan laboratorium dental dapat menangani kekurangan ruang untuk pontik gigi premolar pada daerah edentulous yang sempit.menunjukkan keberhasilan. Simpulan: Perawatan menunjukkan keberhasilan dalam mendapatkan ruang untuk pontik dengan memodifikasi bentuk dan ukuran gigi pendukung.

ABSTRACT

Introduction: Replacing missing teeth aims to maintain the integrity of the stomatognathic system to prevent function alteration due to tooth movement. This case report was discussed the treatment of right maxillary premolar teeth with the narrow edentulous area. Case report:A 20-year-old male patient came to the University of North Sumatra Dental Hospital (RSGM USU) with complaints of mastication difficulty after tooth extraction about 6 months before. Clinical examination showed the loss of tooth 15 with a narrow edentulous area. Radiographic examination showed normal periodontal ligament tissue and adequate bone support of teeth 14 and 16. The diagnosis was the loss of tooth 15 with a narrow edentulous area. The treatment plan was the manufacture of a dental bridge (pontics) with porcelain fused to metal material. The size of teeth 14 and 15 was equated by dividing the distal distance 13 to mesial 16, to get a proportional pontic chamber. Based on clinical examination, supporting examination, treatment plan, and diagnosis, the patient's prognosis was good. The thickness of the planned preparation, diagnostic wax-up, and good communication with the dental laboratory were able to handle the lack of space for premolar pontics in narrow edentulous areas. Conclusion: Treatment shows success in gaining space for pontics by modifying the shape and size of the supporting teeth.
