Abstract


ABSTRAK

Pendahuluan: Periodontitis Apikalis (PA) adalah peradangan jaringan periapikal yang berawal dari lesi karies. Tubuli dentin yang terbuka akibat adanya karies menjadi jalan masuk bakteri dan produknya. Proses ini akan terus berlanjut hingga mengakibatkan nekrosis pulpa dan peradangan berat sampai ke periapikal gigi. Infeksi pada gigi ini akan memediasi peradangan dan menginduksi sel untuk mengeluarkan sitokin proinflamasi. Tumor Necrosis Factor-Alpha (TNF-α) merupakan salah satu sitokin pada respon inflamasi akut bakteri Gram negatif dan protein pada fase akut. Tujuan penelitian menganalisis perbedaan jumlah kadar TNF-α di dalam sirkulasi darah vena antara pasien dengan periodontitis apikalis dan pasien dengan pulpa normal. Metode: Penelitian ini merupakan observasional analitik dengan rancangan cross sectional study. Sampel diambil dari darah vena 10 orang pasien dengan periodontitis apikalis dan 10 pasien normal (tanpa adanya gigi yang terinfeksi PA). Sampel ditentukan berdasarkan hasil anamnesis, pemeriksaan klinis yang dilakukan seperti tes perkusi atau palpasi dan terdapatnya gambaran radiolusen di apikal gigi. Kadar TNF-α, diperiksa menggunakan metode Sandwich ELISA, kemudian data dianalisis menggunakan uji statistik T tidak berpasangan. Hasil: Kadar TNF-α pada sampel darah vena pasien dengan periodontitis apikalis berkisar antara 0,068-0,121 mg/L, sedangkan kadar TNF-α pada pasien dengan pulpa normal berkisar 0,061- 0,143 mg/L. Uji T-test independent menunjukan nilai p=0,389 (p<0,05). Simpulan: Kadar TNF-α pada pasien dengan PA dan pulpa normal tidak memiliki perbedaan yang signifikan secara statistik.

ABSTRACT

Introduction: Apical periodontitis (AP) is an inflammation of periapical tissue initiated by caries lesions. The exposed dentin tubules due to caries are an entrance for bacteria and its products. This process will continue to result in pulp necrosis and severe inflammation extending to the periapical area of the tooth. The infection of this tooth will mediate inflammation and induce cells to release proinflammatory cytokines. Tumor Necrosis Factor-Alpha (TNF-α) is one of the acute inflammatory response cytokines in Gram-negative bacteria and protein in the acute phase. The purpose of this study is to analyze different levels of TNF-α in venous blood circulation between patients with AP and normal pulp. Methods: The type of study was an observational analytic with cross sectional study design. The blood samples were collected from 10 patients with AP and 10 with normal pulp. Samples were determined from medical history, percussion and palpation clinical examination, and apical radiolucency. TNF-α levels were then examined using the Sandwich ELISA method, and data were analyzed using an unpaired T-test. Results: TNF-α levels in venous blood samples from patients with AP ranged from 0,068-0,121 mg/L, and TNF-α levels in patients with normal pulp were  0,061- 0,143 mg/L. Unpaired t-test showed no significant difference p=0,389 (p<0.02). Conclusion: TNF-α levels in patients with AP and normal pulp did not differ statistically.
