Metode kompaksi vertikal panas (warm vertical compaction) sebelum aplikasi pasak fiber: laporan kasus

ABSTRAK
Pendahuluan: Pasak diindikasikan pada gigi pasca perawatan saluran akar pada gigi dengan kerusakan struktur gigi yang luas, namun tahapan persiapan aplikasi pasak dapat menyebabkan berbagai komplikasi. Persiapan pasak meliputi pengambilan gutta-percha yang dapat dilakukan secara mekanis, kimia, atau termal. Metode termal pada teknik kompaksi vertikal panas (warm vertical compaction/WVC) memiliki berbagai kelebihan yang dapat mengoptimalkan persiapan aplikasi pasak fiber. Tujuan laporan kasus ini membahas metode kompaksi vertikal panas sebelum aplikasi pasak fiber. Laporan kasus: Pasien wanita, 62 tahun mengeluhkan gigi depan atas berlubang besar dan sakit berdenyut sejak 2 minggu lalu. Rencana perawatan adalah perawatan saluran akar gigi 12 dengan restorasi pasak fiber dan porcelain fused to metal crown. Teknik WVC digunakan sebagai persiapan aplikasi pasak fiber pada kasus ini. Metode ini menjadi pilihan persiapan aplikasi pasak fiber karena dapat mengoptimalisasi kondensasi gutta-percha, preparasi yang minimal invasif, mengangkat gutta-percha tanpa risiko mengambil dentin, mengurangi risiko kebocoran apikal, membantu penetrasi semen resin dan efisien dalam satu kunjungan. Metode ini umum ditemui pada kasus sehari-hari dan akan meningkatkan kualitas hasil perawatan, namun belum banyak laporan kasus yang menganalisisnya. Simpulan: Metode kompaksi vertikal panas dapat menjadi salah satu pilihan teknik sebelum aplikasi pasak fiber dalam satu kunjungan.

Warm vertical compaction method prior to fiber post application: a case report
ABSTRACT
Introduction: Post was indicated for endodontically treated teeth with extensive tooth structure destruction. However, preparation for post application could cause various complications. Post preparation involved removal of gutta-percha which could be done mechanically, chemically, or thermally. The thermal method with warm vertical compaction (WVC) technique had various advantages that could optimize the preparation of fiber post applications. The purpose of this case report was to discuss warm vertical compaction method prior to fiber post application. Case Report: A 62-year-old female patient complained of a large cavity in her upper front tooth and throbbing pain since 2 weeks ago. The treatment plan was root canal treatment for tooth 12 with fiber post and porcelain fused to metal crown. The WVC technique was used in preparation for fiber post application in this case. This method was the preferred preparation method for fiber post applications because it could optimize gutta-percha condensation, was a minimally invasive preparation, removed gutta-percha without the risk of taking dentin, reduced the risk of apical leakage, helped resin cement penetration and was efficient in one visit. This method is commonly found in everyday cases and will improve the quality of treatment results, but there are not many case reports that analyze it. Conclusion: The warm vertical compaction method could be a technique of choice before fiber post application in one visit.
