Keberhasilan perawatan apeks terbuka akibat resorpsi akar pada previously treated tooth dengan menggunakan MTA: laporan kasus

Abstract


ABSTRAK 

Pendahuluan: Gigi nonvital dengan apeks terbuka merupakan suatu tantangan dalam praktik endodontik. Apeksifikasi adalah metode untuk menginduksi barrier kalsifikasi pada apeks terbuka untuk memfasilitasi obturasi.      Gigi yang telah dirawat endodontik (previously treated teeth) dengan tidak adekuat masih bisa menyisakan bakteri yang komposisinya mirip dengan gigi yang belum pernah dirawat (previously untreated). Tujuan dari laporan ini adalah untuk mendeskripsikan perawatan apeksifikasi pada gigi previously treated dengan resorpsi akar. Laporan kasus: Seorang pasien laki-laki berusia 37 tahun mengeluh nyeri pada gigi #11 dan pembengkakan pada gusi di sekitarnya. Radiograf menunjukkan pengisian non hermetik di saluran akar dan resorpsi akar dengan radiolusensi difus di daerah periapikal. Diagnosisnya adalah previously treated tooth dengan abses apikal akut. Perawatan ulang dilakukan dengan file-H, dressing Ca(OH)2 diberikan sebagai medikamen intrakanal, dan barrier apikal diperoleh dengan menggunakan Material Trioxide Aggregate (MTA). Obturasi dilakukan dengan teknik injeksi termoplastik. Hasil perawatan menunjukkan keberhasilan yang ditandai dengan hilangnya keluhan pasien. Pemeriksaan objektif intraoral dan ekstraoral dalam batas normal. Pemeriksaan radiografi menunjukkan obturasi yang hermetis, tidak ada sisa semen yang tertinggal dan lesi periapikal yang mengecil. Pasien puas dengan hasil perawatan. Simpulan: Perawatan apeks terbuka akibat resorpsi akar pada previously treated tooth menggunakan MTA dalam menginisiasi pembentukan barier kalsifikasi menunjukkan keberhasilan yang baik. Penggunaan MTA dalam apeksifikasi dapat mengurangi waktu yang dibutuhkan untuk menyelesaikan perawatan saluran akar sehingga memungkinkan dilakukannya restorasi akhir lebih dini dan meningkatkan kemungkinan keberhasilan perawatan. Perencanaan perawatan yang terstruktur diperlukan untuk memastikan keberhasilan perawatan.

Successful treatment of open apex due to root resorption in a previously treated tooth using MTA: a case report


ABSTRACT


Introduction: Nonvital teeth with open apices due to root resorption pose a challenge in endodontic practice. Apexification is a method utilized to induce the formation of a calcified barrier at the open apex, aiming to facilitate obturation. Despite previously treated teeth being considered free from infection, in certain cases, residual bacteria with a composition similar to that of previously untreated teeth may still persist. The purpose of this report was to describe apexification treatment in previously treated tooth with root resorption. Case Report: A 37-year-old male patient complained of pain in tooth #11 and swelling in the surrounding gum. Radiographs revealed non-hermetic filling in the root canal and root resorption with diffuse radiolucency in the periapical area. The diagnosis was a previously treated tooth with acute apical abscess. Retreatment was performed using H-files, intracanal medication with Ca(OH)2, and an apical barrier was obtained using Mineral Trioxide Aggregate (MTA). Obturation was performed using thermoplastic injection technique. A successful treatment results marked with the resolution of patient’s complaints. Intraoral and extraoral objective examinations were within normal limits. Radiographic examination indicated hermetic obturation, no remaining cement debris, and reduction in periapical lesion size. The patient was satisfied with the treatment outcome. Conclusion: Treatment of open apex due to root resorption in a previously treated tooth using MTA to initiate the formation of calcific barrier demonstrates a high level of success. The use of MTA in apexification can reduce the time required to complete root canal treatment, allowing for earlier final restoration, thereby increasing the likelihood of treatment success. Structured treatment planning is necessary to ensure treatment success.
